Celtic Insurance in Texas
Products Available:
Celtic Healthy Texas is a group
health insurance plan available to qualifying small
businesses.
CeltiCare is Celtic's best-known
premier coverage for healthy applicants. The most current version of
CeltiCare insurance (5.1) has
been available to individuals
and families in Texas since June
2010.
Celtic Basic is a more affordable
coverage for healthy applicants. The most current version of
Celtic Basic (2.2) has been
available to individuals and
families in Texas since June
2010.
Celtic Saver HSA takes advantage of
the cost-saving features of a Health Savings Account.
High deductible insurance plans are comined with
optional qualified savings accounts available at no
additional cost. The most current version of
Celtic Saver Health Savings
Account plan (3.0) has been
available to individuals and
families in Texas since June
2010.
Celtic Short Term Medical Insurance
is available for 1 month or longer (up to a 6 or 12
month maximum) with more liberal eligibility
requirements. Short term coverage is less expensive
because it does not pay the cost of prior medical
conditions.
The most current version of
Celtic STM (3.0) has been
available to individuals and
families in Texas since June
2010.

State-specific Variations of Coverage
CeltiCare, Celtic Saver HSA and Celtic Basic
(except where noted) health plans have the
following significant
variations in coverage in accordance with state law:

Term Life Option - Not available

Child Immunization - from birth through age 6. Immunization charges not subject to the deductible, coinsurance or copay. Charges for
other services rendered at the time of immunizations are subject to the deductible, coinsurance and copay requirements.

Hearing Impairment – coverage provided for newborn screening test from birth through 30 days of age. Follow-up care provided through
age 24 months. Services not subject to the deductible; however coinsurance, copay requirements apply.

Home Health Care - 60 visits per year.

Prescription Drugs - includes birth control drugs and formulas for treating phenylketonuria or other heritable diseases if prescribed
by a physician.

Off Label Drug Use – coverage provided for treatment of a covered chronic or disabling condition if FDA approved for at least one
indication and is recognized for treatment of the indication in peer review medical literature or a prescription drug reference
compendium.

Psychiatric Care -Also covers charges for: 1) psychiatric day treatment facility. 2) treatment provided through a crisis
stabilization unit. and 3) treatment provided in a residential treatment center for children and adolescents as required by Texas
law.
(Not applicable to Celtic Basic).

Telemedicine and Telehealth Services- covered in accordance with generally accepted health care practices and services.

Colorectal Cancer Screening – covered in accordance with the standards established by the American Cancer Society.

Craniofacial reconstructive surgery – covered for children under 18 years of age.

Acquired Brain Injury – coverage for all integrative cognitive and rehabilitative therapies as a result of or related to an acquired
brain injury.

Home Health Care Charges – Up to 60 visits per calendar year, but only if a hospital or extended care facility confinement would
otherwise be needed.

Child Hearing Impairment Screening Test – from birth through age 30 days. Necessary follow up care until 24 months old.

8/2/2011 Superior HealthPlan Inc. and Superior
HealthPlan Network were awarded a new contract Tuesday that will expand
the company's service area for Medicaid managed care programs in Texas
and add millions in revenue. Superior HealthPlan is owned by Centene
Corporation, the parent company of Celtic Insurance Company. The new
contract is expected to bring $666 to $1.1 billion revenue to the health
plan provider.
5/1/2011 Texas residents using a paper application form should now
use state specific application form
U5-581-00241-TX for all individual medical insurance and state
specific application form
U5-589-00249-TX for short term insurance. A replacement form,
included with the application, is also
required when replacing coverage. The correct application forms are
always automatically used when using the online enrollment system.
