State-specific Variations of Coverage

CeltiCare and Celtic Saver HSA health plans have the following significant variations in coverage in accordance with state law:

• Alcoholism -thirty days of inpatient coverage for the primary treatment of alcoholism in any three­hundred-sixty-five-day benefit period with at least two such inpatient treatment periods available during the lifetime of the certificate, and sixty outpatient treatment visits during the lifetime of the certificate.
• TMJ/Craniomandibular Disorder Charges – Diagnostic services and surgery up to $2500 per lifetime.
• Dental General Anesthesia – coverage is provided for general anesthesia and related facility charges if an insured is developmentally disabled or for a child under the age of eight.
• Newborn Child Coverage – for newly born children of the insured person from the moment of birth. The coverage for newly born children shall consist of coverage of injury or sickness including the necessary care and treatment of medically diagnosed congenital defects and birth abnormalities.
