Celtic Insurance in Arkansas

CeltiCare insurance (5.1) has

and families in Arkansas
since June
2010.
Celtic Basic is a more affordable
coverage for healthy applicants. The most current version of
Celtic Basic (2.2) has been

Celtic Saver HSA takes advantage of
the cost-saving features of a Health Savings Account.
High deductible insurance plans are combined with
optional qualified savings accounts available at no
additional cost. The most current version of
Celtic Saver Health Savings
Account plan (3.0) has been
available to individuals and
families in Arkansas
since June
2010.
Celtic Short Term Medical Insurance
is available for 1 month or longer (up to a 6 or 12
month maximum) with more liberal eligibility
requirements. Short term coverage is less expensive
because it does not pay the cost of prior medical
conditions.
The most current version of
Celtic STM (3.0) has been
available to individuals and
families in Arkansas
since June
2010.

State-specific Variations of Coverage
CeltiCare Preferred, Celtic Saver HSA and Celtic Basic (except the
last three items as
noted) health plans have the following significant
variations in coverage in accordance with state law:
- 10 Day Free Look
- Term Life Option - Not available.
- Contraceptive Coverage.
- Children’s Preventive Health Care - Benefits are payable at specific intervals through age 18.
- Immunizations – not subject to deductible, coinsurance or co-pays.
- Temporomandibular Joint Disorder (TMJ) - covered same as sickness.
- Manipulative Therapy - No annual or lifetime maximum.
- Routine Nursery Care & Pediatric Charges – up to 5 days for newborn child.
- Phenylketonuria – treatment and dietary formula if prescribed by a physician.
- Colorectal Cancer Screening.
- Dental Anesthesia -General anesthesia and associated hospital or outpatient surgical facility
charges for a dependent child under the age of seven or a person with a diagnosed mental or
physical condition or a person with a significant behavioral problem, if deemed medically necessary
by the primary care physician. Any dental plan would be primary.
- Impairment of speech or hearing- charges for necessary care and treatment but not hearing
devices. This shall include those communicative disorders generally treated by a speech pathologist
or audiologist. (Does not apply to Celtic Basic).
- Outpatient Chemotherapy and Renal Dialysis –
including hemodyalysis and peritoneal dialysis.
Musculoskeletal Disorders – coverage for surgical
and non-surgical treatment affecting any bone or
joint in the face, neck or head including TMJ or
craniomandibular disorder if prescribed or
administered by a physician or dentist. (Does not apply to Celtic
Basic).
- Chemotherapy, Specialty Drugs, Bio-Tech
Medications – administered parenterally in an
outpatient setting. The eligible expense is 75% of
the Prescription Benefit Manager’s charge for these
medications. (Does not apply to Celtic Basic).

10/21/2011 Arkansas children's insurance open enrollment
program is designated annually in the month of October. Individuals under 19 years of age will be eligible applicants
for health insurance during this month or if they experience another qualifying event
as defined by state law. 5/1/2011 Arkansas residents using a paper application form should now
use state specific application form
U5-581-00241-AR for all individual medical insurance and state
specific application form
U5-589-00249-AR for short term insurance. A replacement form,
included with the application, is also
required when replacing coverage. The correct application forms are
