State-specific Variations of Coverage

CeltiCare Preferred and Celtic Saver HSA health plans have the following variations in coverage in accordance with state law:

• Colorectal Cancer Screening - in accordance with the current American Cancer Society guidelines.
• Coverage includes examination and laboratory tests for any nonsymptomatic insured person age 50 or over, or for an insured person less than 50 years of age who may be classified as high risk for colorectal cancer.
• Phenylketonuria Treatment Charges- administered under the supervision of a physician.
Coverage includes testing, diagnosis and dietary formulas which are medically necessary for the therapeutic treatment of phenylketonuria.
