avoid using downloaded paper enrollment forms. Downloaded forms, if used, are in PDF format. To download a free reader, visit www.adobe.com.
If you have trouble downloading or opening a PDF document,
try this: open your Adobe program and uncheck
the "Web Browser Integration" box. In Adobe Acrobat 4.0, you
find this by clicking "File | Preferences | General". In
other versions, the keystroke series may be different.

of this page and it will be e-mailed or faxed to you.

Expect to receive a
confirmation by e-mail within 24 hours of receipt of your enrollment
forms.
Insurance coverage is not in force until approved by the underwriter.

A conditional binder of coverage or temporary coverage may be available
during the underwriting period. Please discuss this with your benefits
adviser.
The time required from the submission of an application form
for permanent renewable coverage to delivery of
your ID cards varies from 7 days to 30 days depending on whether additional
information is required by the underwriter after reviewing the initial
application.
Enrollment applications submitted are not always approved as
requested. Many applicants will be ineligible or offered coverage at
higher rates or with modifications to the coverage. Please discuss
this with your benefits adviser.
