A cancellation request must come from the policyholder in writing, be signed and dated. Fax the request to Policy Owner Services at (800) 600-8796. You may wish to follow-up with a telephone call to customer service at (800) 477-7870 within a few days to confirm receipt and processing of the cancellation request.
Alternate cancellation procedures may be included within the policy certificate for cancellation by mail. If cancelling by mail, we strongly recommend using a mailing method that includes a proof of delivery (certified mail or delivery confirmation).
Celtic Insurance will automatically send a Certificate of Creditable Coverage to the address of record listed on the policy at the same time that the cancellation is processed. The certificate is not deliverable by fax or email at this time. You may need to provide this certificate.
